Your time is valuable, and we don't want you spending your first 15 minutes in the clinic filling out clipboards. Every new patient is required to complete our comprehensive intake forms before their initial assessment. This gives our Doctors of Physical Therapy an in-depth understanding of your medical history, injury timeline, and specific athletic goals before you even arrive.

Trigger point dry needling is a highly effective treatment we use to quickly release muscle tightness, alleviate chronic pain, and accelerate recovery. If you and your doctor have discussed incorporating dry needling into your performance plan, please review our information guide and sign the specialized consent waiver below.
